The counseling ministry of Sixth Mount Zion Baptist Temple is established to assist individuals find the mind of God for their situations. To insure that the counseling time is used effectively the following questionnaire is to be completed prior to the counseling session. Please make sure the questionnaire is returned at least two days before your scheduled session. This gives our Pastor arnple time to pray for you and your situation prior to your session.
